- April 6, 2011, MDG Medical Software SIG Presents Medical Device Interoperability and Communications Standards
The days of reading a blood pressure, a glucose value, or even an electroencephalogram from a paper printout (or transcribing from a screen to a paper chart) are nearly over. Software-controlled devices that read or control medical parameters are generally expected to communicate – with other devices or with a hospital network, or some other remote system. Even more critically, devices need to be "aware" of related functions in other devices to protect patient safety (e.g. respirator must pause so the X-ray system can take an image, then the respirator should re-start).
Communications/interoperability regulations and standards are just starting to emerge from regulatory and standards bodies. Industry associations and large corporate players of course are trying to influence the direction to their competitive advantage. Our speakers will review some of the key challenges and current efforts in the medical device interoperability field, and provide their view of the way forward in this essential area.

Just as open source software is gaining wider use in various industries, it is being employed in medical applications as well – such fields as electronic health records, clinical trial data management, and medical devices for therapy and diagnostics. Low cost and easy availability make open source attractive, but FDA-regulated manufacturers face unique challenges. The responsibility to assure safety and effectiveness, and to maintain control over the development lifecycle, lead to tough questions:
- How can manufacturers meet design control requirements?
- What hazards have been introduced by the uncontrolled software?
- How do manufacturers gain the necessary confidence in the software?
- What can manufacturers use to qualify the software?
- If a major issue occurred with an open source component, how will FDA react?
As well, the manufacturer's need to protect intellectual property contrasts with the open source licensing philosophy of free distribution.
The FDA requires full validation of medical device software, regardless of its origin: in-house, bespoke, third party, or open source. In light of proposed changes to the 510(k) process, increasing use of open source software in regulated products could attract greater agency scrutiny.
Our speakers will explore these topics and others, in the context of their own experience with open source software.

MDGent (the Entrepreneur SIG) is sponsoring a brainstorming session for anyone who is working in a start-up company or is even thinking about starting up a company. We invite key people from start-up companies to discuss any issues they are having (regulatory, legal, marketing (and/or sales), scientific, manufacturing, financing, or just about anything else) and would like ideas on how to address those issues. We will have people with specific expertise at the meeting to suggest ideas and, if possible, give referrals to others who can help. This will be an interactive meeting with only a brief presentation in the beginning. Come to this meeting with all of your questions, concerns, or issues, and we will work on them together.

"Legal Challenges for Startups," the first meeting of 2011 for the Boston Entrepreneurs' Network (E-NET), features a panel of attorneys with extensive experience in the legal and business issues faced by high technology companies and their founders. He joined Helicos Biosciences in 2004 as a Business Development Associate, working with the CEO and co-founder Stanley Lapidus to identify target customers and applications for the world's first single molecule sequencing system, the Helicos Genetic Analysis System. November 10, 2010, MDG Medical Software SIG Presents – Embedded Design and Development for Medical Applications
As medical-device manufacturers concentrate their efforts on portability, wireless connectivity, home health, safety, data security, quality/reliability and low cost, they are moving rapidly toward embedded software implementations. At the same time, major suppliers in this area are offering more robust products, tools and services to support embedded development.
Although the lifecycle for embedded software development is very similar to that of conventional software, it differs in a few crucial aspects. Efficiency is much more of an issue, since the software is much "closer" to the hardware than conventional software operating on a general purpose computer. As well, real-time processing often imposes serious constraints.

For some organizations, quality assurance seems like such an alien concept when applied to software, yet FDA requires evidence of following it: best practice and industry standards for software development and testing. (Note the FDA is not the only regulatory body to expect such proof!) Of course compliance is required, but we need to look beyond the agency's expectations to understand how quality can be a benefit to your business. Then, too, there's the issue of regulations and standards: FDA 21 CFR - GPSV – IEC 62304 – IEEE standards – IEC 600601-1-4 – ISO 14971 – and all the other bits in the alphabet soup.

The social media phenomenon is changing the way people connect, but not just for social purposes. In addition to catching up with your high school chums, it is becoming a powerful tool for marketing, monitoring, and directing consumer behavior. The life sciences industries are no exception, and in many ways stand to benefit far more from this communication tool than companies like Starbucks that already have daily face-to-face contact with their customers.
As FDA scrambles to figure out how to regulate a low-level marketing associate at a drug, med device or other life sciences company who tweets about a product on his personal account, companies are faced with the even greater challenge of implementing policies and procedures company-wide that both protect the company and ensure the benefits of the new media.

Special Patients
Despite numerous initiatives such as Patient Centered Medical Home and Neonatal Intensive Care Unit of the Future plus media and government focus on problems and costs of healthcare delivery, infants and late-life adults remain under-served patients populations.
The two populations are excluded from clinical trials due to vulnerability and regulatory concern for the frailty, sensory neurological limitations, and susceptibility of the patients. It is also a reality, however, that there are not enough financial incentives for medical manufacturers to develop products to extend the indications specific to these patient populations.
Experts in the field and stakeholders in the business of geriatric medicine, and pediatric products will discuss the realities, important issues, value proposition, and challenges for treatment and prevention that encompass technology innovation, providers, patient quality of life, as well as regulatory and reimbursement.

- December 10, 2009, Our Current Environment – Emerging Threats and New Security Challenges
Security of biomedical research operations faces evolving challenges and requires adaptive strategies. There continues to be comprehensive physical security needs for plant and personnel. Intellectual property is almost wholly electronic, and protection against cyber-intrusion/cyber-theft is now essential, along with the protection of electronic records containing sensitive personnel, customer, and supplier data. Public companies also must decide how to handle shareholder motions brought by anti-research organizations that have purchased company stock. And hiring, particularly background checking, needs to be conducted in such a way that research staff is protected, as is the research facility itself, against infiltrator employees.
On December 10, 2009, these issues will be addressed at "Our Current Environment: Emerging Threats and New Security Challenges", a day-long professional development workshop offered by The Massachusetts Society for Medical Research. - October 21, 2009, MDG October 2009 Networking: Post-Approval Studies: Maximizing the Value of Clinical Experience
For many people in the medical device industry the term “clinical trials” is most often associated with the trials a company conducts during device development, validation, and as part of obtaining a regulatory clearance.
Additional trials may be conducted in parallel with the FDA review, after a regulatory clearance is issued, and after the device is launched commercially. Such trials may expand the clinical performance data as well as provide evidence on the organizational, financial, and other benefits of integrating a new device in a medical institution.
While these trial results can provide vital information, have a significant impact on the device acceptance by the medical community, and increase its adoption rate, it is crucial to strictly adhere to all the applicable rules and regulations.
This program will provide an overview of clinical trials at various stages following the submission of an application for a regulatory clearance including trial planning, control, identification of data that has to be reported to the FDA, and possible implications for the commercial use of the device.
Format: One 30 minute presentation followed by a moderated Q&A session.
